If you would like to save time at check-in, please fill out the following registration form to bring with you. For your protection, we do check ID on every new client. 

Pet of the Month Form

Would you like to enter your pet to win Pet of the Month? If your pet is selected, you win a free 3 month supply of flea prevention and a $10 credit with Animal Hospital in Fairfield! Just fill out the Pet of the Month form and bring it in or email it back to us at staff@animalvetfairfield.com. Don't forget to bring in or send in a picture as well!

Pet Sitter/Temporary Caretaker Form

Are you going to be away from your pet for a while? If so, please fill out and bring in the Temporary Caretaker Authorization form. Whether you're going on vacation or have to leave on emergency, it's important that we know who you're trusting to care for your pet. Please provide us with as much information as possible so that we may provide the level of service you expect.
